Job summary
An opportunity has arisen for salaried GPs to join our team to work within the Urgent and Emergency Care Centre in Rotherham. We are looking for General Practitioners with commitment, flexibility, enthusiasm, excellent clinical skills, and who are team players.

The trust has vacancies for GPs to work in the Primary Care element of the modern Urgent and Emergency Care Centre. This is a state-of-the-art urgent care facility combining traditional A&E services with Primary Care, including the GP Out of Hours service. Our setting is the first site to go live with a fully integrated NHS pathways streaming and redirection product, work continues in its implementation.

The UECC operates in an innovative and exciting way, using GPs and Advanced Clinical Practitioners to provide primary care consultations, including the provision of a Primary Care Streaming service as patients arrive, ensuring patients are seen by the most appropriate service or clinician. Where possible this includes patients being managed at the first point and provided with self-care advice, or redirection to a more appropriate service.

The model allows for the rapid and flexible management of workload and uses the expertise and skills of GPs to enhance the operational capacity of the UECC. In doing so, it recognises the unique and valuable skill set that GPs can bring to the Urgent Care setting.

Main duties of the job
The UECC is very busy environment with staff from all disciplines working together to apply the latest in current strategies for urgent care delivery. The service is rapidly evolving with the input from all members of the team to provide “Right care, First time”.

The Primary Care team within the UECC brings together primary care and GP Out of Hours services and is GP led with the support of Advanced Clinical Practitioners. The post holder will be seeing all types of patient presentations that attend the department from minor injuries and illnesses to patients with acute or life-threatening conditions, which the post holder must feel confident in being able to assess and refer on as required.

Rotherham Urgent and Emergency Care Centre consists of an integrated workforce; bringing primary and secondary care expertise together to improve patient care. The wider UECC team includes; ED consultants, Middle Grade and Junior doctors, ACPs, ENPs, and ED nursing colleagues.

Job responsibilities

Job Purpose:
The Urgent and Emergency Care Centre operates a Primary Care led front door streaming service, where senior primary care clinicians working in tandem with experienced triage nurses and NHS pathways streaming and redirection tool to perform initial assessment of cases at first presentation, make decisions on appropriate management and disposal; including managing cases by redirection to more appropriate services at the point of initial contact; streaming into a Primary Care Stream, a hospital Same Day Emergency Care facility, or to the Emergency Department triage.

As a valued member of the team, Primary Care clinicians also provide a highly specialist resource to other healthcare staff, patients and carers and have active involvement in Clinical Governance/Effectiveness activities.

Key Result Areas and Responsibilities:

Managerial
To manage the service caseload in partnership with other practitioners to ensure quality outcomes for patients and carers.

To function as an accountable General Practitioner, managing clinical care and facilitating general management through partnership with the service, management team and other specialities.

To produce and comply with a job plan developed in partnership with the GP Clinical Lead (Primary Care & GP Out of Hours) to ensure all practice and other work is delivered appropriately and safely.

Primary Care clinicians will be expected to respond positively to changing workload and demand, working to ensure appropriate timescales for the management of individual cases.

To manage clinical caseload in a diligent and timely manner, through efficient use of clinical time.

Primary Care clinicians will be expected to maintain an overview of the clinical demand and if acting as a designated coordinator, make decisions in partnership with the clinician in charge of UECC and colleagues in the GP Out of Hours service to manage the primary care provision and team to respond to changes in demand.

The post holder is responsible for making sure that any personal equipment used within the UECC, is calibrated and cleaned at appropriate intervals to ensure patient safety.

The post holder is required to take charge of the shift, on a shared basis with other clinicians, to ensure that the Primary Care and GP Out of Hours services meet patient demands.

To attend departmental meetings as required by the role.

Key Internal and External relationships

Information Management
To provide medical leadership as appropriate to service requirements.

To identify and utilise appropriate best practice through benchmarking and clinical guidelines.

To develop positive, collaborative working relationships across all areas of the care team within and associated with the UECC.

To take ownership and provide leadership to the team in specified clinical areas, including training and mentoring of junior members of staff, including GP speciality trainees and trainee advanced clinical practitioners.

To undertake appropriate clinical risk assessments and implement management strategies for risk identified.

To work collaboratively with the GP Clinical Lead for Primary Care & GP Out of Hours and the UECC team to ensure that effective clinical governance is incorporated in the day-to-day delivery of care.

Patient/Client Care

Clinical
To provide appropriate care and services to patients in all ranges and level of health care needs.

To respond to medical/health problems presented by patients including history taking, diagnosis, investigation, treatment and referral as appropriate.

To prescribe generically for patients as appropriate to their need and if non-generics are utilised be able to provide appropriate clinical rationale with support from medicines management team.

To prescribe in a cost-effective manner.

To provide appropriate health education and advice to patients and carers to enable self-management and choice.

Non-Clinical
To maintain clinical records as required, either written or computer based, in line with professional code and requirements utilising appropriate coding within the system.

To work with colleagues to manage the clinical caseload to satisfy current service requirements in line with national and local NHS targets.

To develop and maintain high levels of communication with patients, carers and colleagues ensuring effective care delivery.

To take part in effective communication processes between all team members to enable good working practices.

To ensure communication processes with acute Trust services, other colleagues and partners in care to ensure patient centred care is delivered.

Research and Development
To be actively involved in developmental processes within the service, in partnership with other health and social care professionals as appropriate.

To deliver service development requirements in the UECC as agreed with colleagues, managers and TRFT Strategic Plan.

To share and disseminate acquired knowledge with all the UECC team through development / departmental meetings including clinical governance and incident reviews.

To develop and implement, as appropriate, guidelines, policies and procedures as required.

To actively engage in effective communication with UECC managerial support processes to ensure equity of service delivery and target achievement.

Working Conditions
The Urgent and Emergency Care Centre is a purpose-built facility incorporating all aspects of Urgent and Emergency Care. The unit is busy in line with most Urgent Care facilities and incorporates primary care services as part of the 24-hour commitment to Urgent Care.

The primary care element of the service operates a primary care led streaming service. A member of the team will be designated coordinator for each shift and will be expected to maintain an overview of the primary care stream, liaising with colleagues across the department to maximise efficiency and proactively manage demand.
